Key Features of the HealthMapRx\u2122<\/strong>\u00a0<\/sup>Program:<\/strong><\/h5>\n

Program participants are paired with a licensed Pharmacist Care Manager and meet face to face, in private, and by appointment 4-6 times per year, usually at the worksite and during work hours.\u00a0 Most sessions are scheduled for 30-45 minutes.\u00a0 Participants receive incentives for participation and for working with their Pharmacist Care Manager to meet basic program requirements.[\/vc_column_text][vc_empty_space][vc_column_text]<\/p>\n

During each appointment, the Pharmacist Care Manager focuses on:<\/strong><\/h5>\n
    \n
  • Optimization of drug therapy, including promoting adherence<\/li>\n
  • Assessment and improvement of participant knowledge and self-care skills<\/li>\n
  • Establishment of personal goals relevant to health risk priorities<\/li>\n
  • Providing motivation and support for the achievement of goals and a healthier lifestyle<\/li>\n
  • Preventative care \u2013 identification and management of \u201ccare gaps\u201d vs. accepted standards<\/li>\n
  • Communication and coordination with the participant\u2019s medical provider(s)<\/li>\n
  • Accountability – for participants and Pharmacist Care Managers in adhering to program requirements<\/li>\n<\/ul>\n
