HealthMapRx™ is a chronic condition care management program designed to reduce financial and clinical risks associated with prevalent chronic conditions, including Pre-Diabetes, Diabetes, Cardiovascular Disease, Asthma, and Behavioral Health. HealthMapRx™ reliably produces savings for our customers through reduced hospitalizations and ER visits, as well as avoidance of preventable complications commonly associated with these conditions.
Key Features of the HealthMapRx™ Program:
Program participants are paired with a licensed Pharmacist Care Manager and meet face to face, in private, and by appointment 4-6 times per year, usually at the worksite and during work hours. Most sessions are scheduled for 30-45 minutes. Participants receive incentives for participation and for working with their Pharmacist Care Manager to meet basic program requirements.
During each appointment, the Pharmacist Care Manager focuses on:
- Optimization of drug therapy, including promoting adherence
- Assessment and improvement of participant knowledge and self-care skills
- Establishment of personal goals relevant to health risk priorities
- Providing motivation and support for the achievement of goals and a healthier lifestyle
- Preventative care – identification and management of “care gaps” vs. accepted standards
- Communication and coordination with the participant’s medical provider(s)
- Accountability – for participants and Pharmacist Care Managers in adhering to program requirements
