Group Education

Weight Management:

PPCN offers a voluntary weight management program including weekly small group sessions, delivered in-person or virtually, over a three-month period.  The intent of the program is to provide education, support, and facilitated group discussion for those whose personal wellness goals include weight loss or weight maintenance.  The development of healthy habits to achieve a long-term impact on health and well-being is emphasized during each session.

Services may include, but are not limited to:
  • Initial participant survey requesting education topics of choice and availability to establish the program schedule
  • Multiple sessions prior to the start of the weight management period designed for registration, orientation, and instruction using specific applications and/or additional resources
  • Weekly customizable support sessions, lasting approximately 30-45 minutes
  • The option of select sessions delivered by Registered Dietician
  • Execution of administrative tasks, documentation, reminders, participant support, and aggregate reporting
  • Participant satisfaction survey at the end of the program cycle
Alternative Wellness Participation Options:

On behalf of employers with programs that are legally obligated to provide an alternate voluntary participation option, PPCN offers individual or group sessions designed to meet the customer’s needs.  The intent of this program is to deem qualifying participants eligible for an employer-paid benefit upon successful completion of designated program requirements.  A typical program includes weekly one-hour instructional sessions over the course of six weeks.  Topics generally focus on nutrition or other modifiable lifestyle habits that help the participant achieve a healthier outcome.  Program content and schedule can be modified to the customer’s unique specifications.
